Terms and Conditions 

1     Introduction 

1.1 These Promotion Specific Terms (the “Promotion Specific Terms”) contain the specific terms and conditions which govern the “Survey #LL190925 (Unredeemed tickets survey)” (the “Promotion”). By opting in to this Promotion, a Player, Prospective Player, Intending Player, or any other user (as applicable) (all the foregoing, hereinafter an “Individual”) is confirming that they understand, accept, and agree to be bound by these Promotion Specific Terms which govern this Promotion.

1.2 This Promotion is subject to the General Terms and Conditions which may be found here. The General Terms are hereby incorporated into and form part of these Promotion Specific Terms. Any conflict or inconsistency between these Promotion Specific Terms and the General Terms shall be resolved in accordance with the General Terms. Any capitalized terms not otherwise defined herein shall have the meaning ascribed to them in the General Terms.

1.3 The General Terms contain the terms and conditions that govern, among other things, Merged Bonuses, Wagering Requirements, Wagering Periods, and Qualifying Requirements. If an Individual does not agree with these Promotion Specific Terms or the General Terms, they must (i) not opt in to the Promotion; or (ii) immediately opt out of the Promotion in accordance with Section 4.2.

1.4 This Promotion is intended to be conducted separately and with a distinct set of Promotion Specific Terms from all other promotions being administered by OLG.

2     Promotion Eligibility

2.1 The Promotion is available in Ontario only and only to those residing in Ontario who (at the time of participation):

1. are eighteen (18) years of age or older; and
2. have a valid Player Account that is in good standing in accordance with the Player Agreement and has not been archived, self-excluded or registered for My PlayBreak or deactivated; and

3. have been selected to participate in this Promotion and are otherwise qualified and not precluded from participating in OLG marketing and promotions, including the Promotion, based on the application of OLG’s current policies at the time of the Promotion, including OLG’s responsible gambling policy (“Eligible Participant”)

   Without limiting the foregoing, an Individual will no longer be eligible to participate in this Promotion or receive any Bonus Funds or other substitute benefit if that Individual is rated as high risk under 3.10 of the Responsible Gambling and My PlayBreak Policy at the time Lottery Bonuses are issued to Eligible Participants as part of this Promotion, regardless of whether that Individual received an invitation to participate in this Promotion.

   Volunteers, employees, officers and directors of OLG, the Alcohol and Gaming Commission of Ontario and any of OLG’s advertising, promotion and contest management agencies are not eligible to participate in this Promotion.

2.2 In the event that an Individual does not meet the eligibility requirements as set forth herein, OLG has the right, in its sole discretion, to reject or disqualify the Individual from participating in this Promotion. The disqualified Individual will not be entitled to any Bonus Funds or other substitute benefit due to such disqualification.

3     Promotion Period

3.1 This Promotion is available as of September 24, 2025, at 10:00am ET to October 3, 2025, at 11:45pm ET (the “Promotion Period”). The Eligible Participant must opt in to this Promotion by COMPLETING THE SURVEY during the Promotion Period or before the survey is closed.

4   How to Participate

4.1 To opt in to the Promotion, Eligible Participants must complete the following steps during the Promotion Period:

1. Click on “START SURVEY”; and
2. Complete the survey in full to be eligible for the $5 Lottery Bonus

4.2 If the Eligible Participant no longer wishes to participate in this Promotion after they have completed the steps set out in Section 4.1, they must opt out by contacting Player Support through the “Contact Us” page or by calling 1-800-387-0098. Reference “Survey #LL190925”

5   Qualifying Requirements

5.1 Subject to Section 5.2, Eligible Participants who opt in to the Promotion in accordance with Section 4.1 must complete the following steps by October 3, 2025, or before the survey closes, whichever comes first (“Qualifying Period”) to qualify for a $5 Lottery Bonus (while quantities last):

1. Complete and submit the survey in full via the provided link. The survey must be completed in full before the deadline i.e. October 3, 2025. The survey may close earlier than the stated deadline if the target number of responses is reached.

5.2 There are five hundred (500) $5 Lottery Bonuses available for redemption as part of this Promotion. Lottery Bonuses will be issued on a first-come, first-served basis, in accordance with section 5.1. Eligible Participants understand and acknowledge that completion of the steps in Section 5.1 is entirely voluntary and that doing so does not guarantee receipt of any Bonus Funds or substitute benefit. If an Eligible Participant does not satisfy the requirements set out in Section 5.1, the Eligible Participant will not be entitled to any benefit or compensation of any sort.

6   General Terms

6.1         Eligible Participants may only redeem the Promotion one (1) time.
